Registration Form
As part of the Registration Form, we collect personal data necessary to schedule an appointment and properly prepare the Psychological Consultation, including: first name, last name, email address, phone number, residential address, date of birth, age, education, marital status, occupation, reason for seeking support, and the contact details of a person to be notified in case of a sudden health emergency.
These data are processed solely for the purpose of registration and appointment organization, communication regarding the scheduled session, and provision of appropriate psychological support.
